Waiver and Release
In consideration of the foregoing, I waive and release any and all rights, claims and courses of action I have or may have against New Faith Temple C.O.G.I.C. The Event, Stepping into a New Season walk-a-thon, and any and all sponsors or other representatives, that may arise as a result of my participation in The Event.  I attest and verify that I am in sufficient good health and physical condition to participate in The Event.  Further, I hereby grant full permission to any of the foregoing to use any photographs or any other record of this event for any legitimate purpose.
